LIGHT OF HOPE FAMILY PROGRAM
Thank you for your interest in Bringing Hope Home’s Light of Hope Family Program. If you have a Family that is in need of financial assistance as they battle cancer, we want to help. Please review our guidelines to make sure the family that you wish to refer meets our requirements before applying.
If this is your first time referring a Family to Bringing Hope Home, please contact Bringing Hope Home’s office at 484-580-8395 for information on how to apply.
APPLICATION DETAILS
Applications must be submitted online by a social worker or other members of the Family’s oncology team. Applications submitted from any other source will not be accepted. Each hospital, in our serving area, is limited to two referrals per month.
We will accept completed applications online on the following dates between 8 am and 4 pm:

BEGIN APPLICATION BELOW
To start an application for a Family, please enter the password below and click “Access” followed by “Apply.” If you are a social worker or healthcare professional and this is your first time referring a family to BHH, please call our office to request the password. Once the password has been entered, this will open our pre-application form, which should be filled out by the social worker to begin a Family’s application. The pre-application confirms that the Family applying meets our guidelines. A pre-application can be completed and submitted to BHH at any time of the month to begin the family’s application in our system. Please print a HIPAA form for the Family to sign and complete. This form will need to be signed and uploaded on the Family’s pre-application form.
Once the pre-application has been submitted, it is reviewed by BHH’s Family Department. The social worker will then receive an email from a BHH staff member with a link to complete the Family’s full application, if the Family meets our guidelines for assistance. The application can be saved in preparation for submission on the first business day of the month.
We will accept completed applications online on the dates listed above, between 8am and 4pm. Social workers will receive an email confirmation once the Family’s application was accepted and approved.

HOSPITAL PARTNERSHIP PROGRAM
BHH is now offering a Hospital Partnership Program that allows hospitals and cancer centers to refer additional Families in need to the Light of Hope Family Program for support. Through this partnership, social workers can increase the number of Families that they can refer to BHH’s program. To learn more, please click here.
