Guidelines

To qualify for the Light of Hope Family Program, Families must be referred by their social worker at the hospital they’re having their cancer treatment and must:

 

1

Live in one of the following States/Counties we serve

1.  Connecticut: Fairfield County

*Receiving treatment at Danbury Hospital, part of Nuvance Health and Stamford Hospital – Bennett Medical Center

2.  Delaware: Kent, New Castle & Sussex Counties

*Receiving treatment at any hospital or cancer center

3.  Maryland: Anne Arundel, Baltimore, Baltimore City, Carroll, Harford, & Howard Counties.

*Receiving treatment at LifeBridge Health facilities that include Carroll Hospital, Northwest Hospital, The Herman and Walter Samuelson Children’s Hospital at Sinai, and Sinai Hospital of Baltimore, Ascension St. Agnes Hospital, Baltimore VA Medical Center, Medstar Cancer Network, Medstar Cancer Network Loch Raven Campus, and University of Maryland Medical Center.

4.  Maryland Shore: Caroline, Cecil, Dorchester, Harford, Kent, Queen Anne’s, Somerset, Talbot, Wicomico & Worcester Counties.

*Receiving treatment at the Richard A. Henson Cancer Institute, Tidal Health, TidalHealth Nanticoke, and University of Maryland Shore Regional Cancer Center.

5.  New Jersey: Atlantic, Burlington, Camden, Cape May, Cumberland, Gloucester & Salem Counties

*Receiving treatment at any hospital or cancer center.

6.  Northern New Jersey: Bergen, Essex, Hudson, Mercer, Middlesex, Morris, Passaic, Somerset, Union & Warren Counties.

*Receiving treatment at Astera Cancer Care, Hackensack Meridian Health, Hunterdon Hematology Oncology, Hunterdon Regional Cancer Center, Memorial Sloan Kettering Cancer Center Bergen, and Rutgers Cancer Institute of New Jersey.

7.  New York: Bronx, Dutchess, Orange, Putnam, Rockland, Westchester Counties

*Receiving treatment at Memorial Sloan Kettering Cancer Center – Westchester, Memorial Sloan Kettering – Rockefeller, Montefiore Medical Center, Mount Sinai Cancer Center, Mount Sinai Morningside, New York Cancer & Blood Specialists, New York Presbyterian Hospital – Manhattan, New York Presbyterian Hudson Valley Hospital, Northern Westchester Hospital, NYU Langone Health, Phelps Hospital, Ruttenberg Treatment Center, The Blavatnik Family Chelsea Medical Center of Mount Sinai, The Mount Sinai Hospital, Valley-Mount Sinai Comprehensive Cancer Care, and Westchester Medical Center.

8.  Pennsylvania: Berks, Bucks, Chester, Delaware, Montgomery & Philadelphia Counties

*Receiving treatment at any hospital or cancer center.

     Pennsylvania: Lehigh Valley & surrounding area

*Receiving treatment at Lehigh Valley Topper Cancer Institute (Cedar Crest, Muhlenberg, Hecktown Oaks, Carbon, and Pocono campuses)

*Receiving treatment at Penn State Hershey Medical Center or University of Pittsburgh Medical Center & living in one of the following counties: (Adams, Carbon, Centre, Cumberland, Dauphin, Franklin, Fulton, Huntingdon, Juniata, Lancaster, Lebanon, Lehigh, Mifflin, Monroe, Northampton, Northumberland, Perry, Schuylkill, Snyder, Union, & York Counties)

Have a cancer diagnosis and undergoing active treatment at the time of referral. Active treatment includes currently receiving chemotherapy, radiation, immunotherapy, or stem cell therapy. BHH also assists Families who have had a recent bone marrow transplant or recent cancer surgery within the past two months, or who are receiving hospice care.

Be referred by a member of their oncology team. This can be a social worker, clinician, or healthcare professional.

Be advised, this is one-time financial assistance that is used to pay household bills such as rent, mortgage, electric, gas, oil, water, home & cell phone, cable, car payment and car insurance. Funds are paid directly to vendors and not given directly to the Family.

Bills we cannot assist with:  Credit Card, Medical Bills, Personal Taxes, Property Taxes and School Taxes.

Have a financial need that is directly related to their cancer diagnosis.
